Instructional Content Opt-Out Form

Board Policy 6141.2: Recognition Of Religious Beliefs And Customs

Per district policy, parents/guardians may, using the board-approved instructional content opt-out form, opt their students out of instruction on the basis of their religious beliefs.

When submitting the written request to the school site principal, parents/guardians must include:

  1. The specific instructional content to opt out of.
  2. The specific religious belief, custom, or practice it substantially interferes with.
  3. How it interferes, including any relevant grade level or individual student characteristics.
